Our dedicated Rehabilitation Unit, located in an exclusive part of the facility, is designed for those discharged from the hospital, but require short-term skilled rehabilitation and nursing services for a successful transition back home. Our team consists of highly-trained rehabilitation and nursing staff with a focus on short-term transitional goals, as well as long-term quality of life needs for permanent placement. This philosophy is also demonstrated with the separate area reserved for transitional, short-term stay. The increased frequency of daily therapy treatment encounters per week maximize functional abilities and decrease your time away from home. Twin Oaks Nursing and Convalescent Home provides clinically proven therapy programs to help you recover from many of the most common surgeries, procedures, and illnesses. While here, you will have access to therapy services up to 7 days per week to get you home faster. We also offer a full range of skilled nursing services and for long-term care stays as well. Twin Oaks Nursing and Convalescent Home has been serving St. John Parish and the Greater New Orleans area since 1975. We are centrally located in the heart of Laplace, Louisiana.
