HOME CARE TEAM

RN Case Manager: This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.

Skilled Nurse: Assists the RN Case Manager in carrying out the treatment plan, providing care, education, and monitoring the condition of the patient.

Rehabilitation: Including Physical, Occupational, and Speech therapists who work closely with caregivers and patients to achieve a higher level of independence.

Behavior Health Specialist:  an RN who coordinates and manages care with the physician and other disciplines involved.

Medical Social Worker: Assists patients and families with accessing financial resources, facilitates emotional and community support, provides counseling, and helps with transitioning to a new environment such as assisted living or a move into a relative’s home.

Certified Home Health Aide: Assists the patient with bathing, grooming, ambulation and the activities of daily living.
